PHI handled for client practices is governed separately by our Business Associate Agreements.

Scope

This policy covers information collected through the FlowClaim RCM LLC website, flowclaimrcm.com, including the contact form. It does not cover protected health information (PHI) handled on behalf of client practices, which is governed by our Business Associate Agreements and described on our HIPAA compliance page.

Information we collect

When you submit the contact form, we collect the information you provide: your name, practice or organization name, email, phone number, specialty, and any details you add in the message field. The form also records the browser user agent and the page the enquiry came from, which helps us spot automated spam. We do not run analytics or advertising trackers on this site, and we do not set tracking cookies. Our hosting provider keeps standard server access logs, which include IP addresses, for security and troubleshooting.

How we use it

  • To respond to your enquiry and schedule a discovery call.
  • To prepare a billing audit if you request one.
  • To maintain records of business communications.

We do not sell or rent visitor information to third parties.

Do not submit patient health information

The contact form is for practice-level business enquiries only. Do not include any patient names, medical record numbers, or other protected health information in the form or in an email to us before a Business Associate Agreement is in place.

Data retention

Contact form submissions are retained for as long as needed to respond to the enquiry and maintain business records, and are stored on secured infrastructure.

Cookies

This site does not use tracking or advertising cookies beyond what is required for basic site function.
